If you have been a patient of ours already you should have received an email from our office inviting you to claim your account within our new software. (We sent the email around 1pm on November 6th, 2020 and it may have found its way into your spam folder.)

If you somehow didn't receive it, please send us a quick email at general@activespineandsport.com and give us your current email address and we will resend you our "Welcome Email" to help you log in to your existing account and to set up your appointments online through out new software.
